You can refer yourself to the following services on the number above:

  • Physiotherapy
  • Adult Social Services
  • Continence Advice
  • Occupational Therapy

3. Podiatry / Chiropody:

Pick up a self-referral leaflet at the Windmill Practice reception to see whether you are eligable to have NHS podiatry treatment on your feet.

4. Nottinghamshire Fit for Work Service- Support in Work

Do you have a health condition that stops you from working? Advice, support and treatment are available now for residents of Nottingham City. Case Managers work with individuals to help them return and stay in work. The Fir for Work Team support people to manage their health work and their work/life balance. They also signpost to external support services such as debt advice, legal advice and housing support. To find out more, or to refer into the service call 0116 285 1710

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter
